Enrolment Procedures
In order to enrol at Brighton Primary School it is essential the Enrolment Form is completed and forwarded to the school with the relevant supporting documents. To make your enrolment as simple and quick as possible, please ensure you follow these steps. In Zone Children A child must turn 5 years of age by the 30th April in the year of school entry. In order to confirm your child’s enrolment, the confidential enrolment form must be accompanied by the following documents: Ø Immunisation History Statement. For children under 7 who are registered with Medicare, this statement can be obtained by calling Medicare on 1800 653 809, by lodging an online request at http://www.medicareaustralia.gov.au/yourhealth, or by calling into any Medicare Office. Ø Evidence of proof of birth date – a copy of Birth Certificate or Passport details. Ø Proof of residency within the school zone ie. Rates notice, lease agreement etc. Ø Copy of Visa status (if applicable). Please note: “Emergency Contacts” are contacts other than parents. Correctly completed forms, accompanied by all documentation above will ensure your child’s AUTOMATIC ENTRY TO THE SCHOOL provided his/her place of residence is within the school Neighbourhood zone – see the Location Map. Incomplete application forms and forms submitted without the relevant documentation as listed above will be returned by post, which will delay confirmation of your child’s enrolment to Brighton Primary. Children residing outside Brighton Primary School Zone. The enrolment procedure for children whose main residence is outside the school’s Neighbourhood zone is as follows: Parents should complete an Expression of Interest form, which can be collected from the school office. The completed form should be returned to the school by Friday 7th August 2009 accompanied by the following documents: Ø Immunisation History Statement. Please ensure the Melways reference is entered on the Expression of Interest form. ENROLMENT FORMS OR EXPRESSION OF INTEREST FORMS FOR 2010 SHOULD BE RETURNED TO THE SCHOOL OFFICE BY 4.00PM ON 7TH AUGUST 2009. |
